People with diabetes who use GLP-1 receptor agonists like semaglutide are more likely to develop neovascular age-related macular degeneration, a new study finds. https://www.healthday.com/healthday-tv/diabetes/prolonged-use-of-glp-1-receptor-agonists-may-increase-the-risk-for-a-serious-eye-condition People who use popular diabetes drugs called GLP-1 receptor agonists may be at increased risk for a serious eye disease, a new study suggests. Researchers have found a link between prolonged use of GLP-1 medications and neovascular age-related macular degeneration or wet AMD. Wet AMD occurs when blood vessels leak fluid into part of the retina called the macula – causing blurred or reduced central vision. The researchers analyzed health data from 139,000 patients with diabetes who were 66 years of age or older. More than 46,000 of them were taking a GLP-1 receptor agonist – most often semaglutide. The others had no exposure to the drugs. After three years of monitoring, the results showed patients in the GLP-1 group were twice as likely to be diagnosed with wet AMD. The researchers say additional studies are needed to confirm these findings and to better understand the benefits and risks of these drugs. The author of an accompanying editorial says, while GLP-1s have been a tremendous help for people with diabetes and obesity, ”Prescribing physicians need to keep in mind the real and serious ocular adverse events that may occur.” He says more work is needed to determine if people using GLP-1 receptor agonists for weight management or other indications face a similar risk. Why does Turmeric work to help control weight and even fight inflammation? Scientists aren't 100% sure yet, but research suggests that it is probably due at least in part to a strong antioxidant; the polyphenol curcumin. This strong antioxidant has been demonstrated to help Type 2 diabetics lower insulin resistance and lose the weight that can contribute to their disease. Turmeric is rich in curcumin. Even More Evidence of Turmeric's Effectiveness Another study demonstrating the promise held by the spice was conducted at Tuft's University. It was published in the Journal Science Daily in early 2009. This new study reveals stronger evidence that it is fact the curcumin found in turmeric that is the key compound responsible for the weight control. The study looked at not the spice, but the curcumin extracts and discovered that these extracts have a positive effect on laboratory mice losing fat. Their findings conclude that one possible mechanism for the observed weight loss is that the curcumin actually restricts the blood supply to fat cells, halting them from excessive growth. Of course, the jury is still out until more tests can be completed, but it appears the turmeric, a simple grocery store spice can definitely help you to lose fat and lower your chance of contracting weight related illnesses, not to mention look a heck of a lot better. In both genders, leptin response was higher after the carbohydrate meal than after the fat meal and while fasting. However, the following study found that leptin levels need to be low in order for fat burning to take place. So trying to spike your leptin right before a fast could be counterproductive. Leptin & Fat Burning: To keep the human brain supplied with energy when food was scarce, mammals evolved the ability to switch from burning carbohydrates to burning fat in order to preserve skeletal muscle that would otherwise be metabolized and converted to glucose. Scientists long believed that the transition to fat metabolism was instigated solely by a drop in insulin. But a new study identified leptin as a key mediator - during starvation, plasma leptin levels fall, activating a pathway that promotes the breakdown of fat and mediates this critical shift from glucose to fat metabolism. While a drop in insulin also occurs, a decrease in leptin is also necessary for this process to happen. Study Summation: Researchers investigated the rate of carbohydrate and fat metabolism as rats went from a fed to a fasted state. The researchers observed that as the rats fasted, leptin levels decreased and activated a pathway that led to fat burning rather than carb burning. During fasting, the rats' use of stored glucose went down and sugar levels decreased as a result. In turn, levels of insulin and leptin decreased - the rats' bodies then broke down body fat to be turned into ketones for energy. Study – Cell: Researchers had rats fast for 48 hours - plasma non-esterified fatty acid (NEFA), glycerol, and bHB (β-OHB) concentrations increased progressively throughout the fast. Unsurprisingly, the reduction in plasma glucose concentrations that occurs as a fast progresses was accompanied by reductions in plasma insulin concentrations and increases in plasma glucagon concentrations. However, they also found that plasma leptin concentrations decreased by 80% between fed and 48-hr fasted rats. Next researchers specifically investigated the impact of reductions in net hepatic glycogenolysis on rates of endogenous glucose production, plasma leptin concentrations, and activation of the HPA axis. They treated post-absorptive rats fasted for 8, 16, and 48 hour with a small molecule inhibitor of glycogen phosphorylase, which resulted in reductions in plasma glucose and insulin concentrations. This was done as it’s consistent with the negligible contributions that hepatic glycogenolysis would be expected to have on plasma glucose concentrations in the glycogen-depleted state (during prolonged fasting.) Interestingly, the reductions in plasma glucose and insulin concentrations in fasted rats treated with the glycogen phosphorylase inhibitor were associated with reductions in plasma leptin concentrations and increases in HPA axis activity, as reflected by increases in plasma corticosterone and ACTH concentrations in these groups. The researchers next infused rats fasted for 48 hours with leptin and found: Increased leptin levels resulted in the suppression of plasma NEFA, glycerol, β-OHB, and lactate concentrations - suppressed HPA axis activity and plasma corticosterone levels as well.
